The Mission

​The mission of the Nurse Case Manager is to serve as the clinical cornerstone and healthcare guardian of our client’s patient-centered delivery system across the continuum of care. In this role, you will lead, analyze, and optimize multidisciplinary patient care plans, ensuring the safe, efficient, and effective transition of individuals through the healthcare system. You will act as the vital bridge between medical providers, insurance parameters, and patient families, driving the clinical, compliance, and utilization efficiencies that deliver exceptional, lasting health outcomes for the community.

Scope of Impact

  • Clinical & Operational Excellence: Direct, improve, and oversee core patient case management activities—specifically complex care planning, discharge tracking, and continuous clinical evaluation. Coordinate seamlessly with physicians, nursing staff, and social services to ensure flawless care delivery from hospital admission to post-acute handover.
  • Code & Compliance Integrity: Collaborate directly with third-party payers, medical directors, and regulatory representatives to ensure absolute compliance with the Nurse Practice Act, hospital safety practices, infection control policies, and third-party reimbursement regulations. Rigorously enforce patient rights and age-specific care standards.
  • Project Control & Quality: Conduct proactive interdisciplinary site meetings, author precise clinical progress documentation, and maintain meticulous medical records, utilization metrics, and safety assessments to give healthcare stakeholders full transparency into patient status and care progression.
  • Schedule & Resource Leadership: Identify and resolve care plan bottlenecks, complex discharge barriers, and reimbursement issues, spearheading on-the-floor problem solving to maximize patient throughput and hit critical milestones for safe, timely transitions.

License and Experience Requirements

  • Valid Nevada Registered Nurse (RN) License is strictly required.
  • Active Basic Life Support (BLS) certification from the American Heart Association (AHA) is strictly required.
  • Minimum of 3 years of clinical nursing experience in an acute care hospital setting.
  • Minimum of 3 years of case management experience, specifically in Case Management, Discharge Planning, or Utilization Review.
  • Preferred Assets: Recent, documented acute care experience, Pediatric case management experience, or recent Emergency Department / Utilization Review experience is a major plus.
  • Professional Affiliations (Strongly Preferred): Certified Case Manager (CCM), Accredited Case Manager (ACM), or active membership in CCMC, ACCM, or ACMA.

Physical & Environmental Demands

  • ​Regularly required to navigate active clinical units, stand, walk, bend, and utilize medical tools or computers to evaluate patient data and system efficiencies.
  • ​Ability to safely lift and move up to 35 lbs independently. (Following site safety guidelines: items 35–50 lbs may be lifted solo if comfortable, while loads over 50 lbs strictly require team assistance or mechanical lifting devices).
  • ​Comfortable working in an active, high-volume healthcare environment near medical equipment, with exposure to shifting high-stress clinical situations, infection control variables, and moderate noise levels.
